Home Health

We are a home health agency that has been providing quality care since 1968 (only Medicare Certified Public Home Health Agency in Georgia, operating since 1968). We are certified and licensed by the State of Georgia for Medicare, Medicaid and Community Care. We employ professionally trained, licensed, certified nurse, therapists, social workers and aides. We serve the 17-county area each day of the year and to ensure close support of our patients, we maintain 17-county offices staffed by resident professionals.

The mission of Health Department Home Care Services is to assist Georgians in achieving their highest levels of health, development, independence and self-sufficiency through the provision of quality broad-based home care to acutely and chronically ill, disabled and homebound clients in Southeast Georgia.

Health Department Home Care Services is dedicated to helping people who need health care regardless of age, race, sex, religion and national origin. It is the goal of this Agency to deliver the best quality care to citizens in their homes.

To receive home care services you must be:

1. Referred by a physician

2. Confined to your home (some exceptions for Medicaid, Community Care, Private Insurance, etc.)

3. In need of nursing, physical therapy, speech therapy or occupational therapy

4. Referred by Community Care Program

Services available are: Nursing Care, Speech Therapy, Physical Therapy, Occupational Therapy, Home Health Aide, Medical Social Worker and Personal Support Services.

Last year we served in excess of 2,800 patients and did over 100,000 skilled visits and over 175,000 half hour personal support units.
